This form is to help you maintain a submittal score of 4 +

“Submittal Quality – Evaluation of the completeness and consistency of the LEED Application 5 (Excellent) – Perfect or near perfect work, no clarifications required 4 (Good) – Less than 5 documentation issues that require clarification 3 (Fair) – 5-10 documentation issues that require clarification 2 (Poor) – More than 10 documentation issues that require clarification 1 (Very Poor) – Widespread documentation issues, unable to be reviewed” – From GBCI Green Rater Grading 

Each item missed here below will result in 1 point penalty

You will work with the pre qad on 2 rounds of submission after that, your project will be sent to the QAD for final review, and anything not corrected will be counted against your score.

Completed and functioning LEED for Homes Workbook?
Please do not submit this form until GBCI has been paid unless GHI is retained to pay this fee. You must also upload all GBCI certification receipts to the final folder in box
Do all prerequisites say the word verified on the 2nd tab of the workbook?
Are there initials and dates on all the Yes credits and Prereqs?
Project Info Tab filled out correctly?
Is the GBCI Certification fee paid?

Must explain why if project passed 60 days since final. Must get approval if passed 2 years!
HSA Tab Filled out correctly?
Do square feet and bedroom counts all match?
Must match on HERS, Plans, LEED Online, Checklist and Water Performance Calcs (if pursuing)
Is there a plans folder in the final with all these items?
- Site Plans - Floor plans - Elevations - Specifications
4 Photos of building from each side in final folder?
Water Management Checklist Rev 7 beyond uploaded to final and builder signed?
Documentation and photos on box for each category - ideally with links on the checklist to each file
All photos contain date stamps / geo coding?
Required on 2020 and beyond registered projects
Lowrise and Single Family Energy Reports Uploaded?
Are all these included and uploaded in the final folder?
These items may or may not apply but check them if they do and you have them
Must be in the finals folder or they won't be counted.
